Jardiance (empagliflozin) is a brand-name SGLT2 inhibitor used for Type 2 diabetes and heart failure. It's an effective medication — and an expensive one. Without insurance, a 30-day supply of Jardiance retails at $500 to $600. Even with insurance, copays can run $50 to $150 per month depending on your plan's formulary tier.

The good news: Eli Lilly, the manufacturer, offers several programs to bring that cost down significantly. Here's how to access them.

Eli Lilly's Savings Card Program

Eli Lilly offers a manufacturer savings card for commercially insured patients that can reduce Jardiance copays significantly. Eligible patients with commercial insurance can pay as little as $10 per month for up to 24 months. The card covers the difference between your insurance copay and the reduced price, up to a maximum annual benefit.

To qualify, you need a valid prescription for Jardiance, commercial (private) insurance that covers Jardiance, and you cannot be enrolled in any government-funded insurance program (Medicare, Medicaid, Tricare, VA). You can apply online at the Jardiance or Eli Lilly website, and the card is usually activated immediately.

Jardiance Patient Assistance Program (Lilly Cares)

For uninsured patients or those who can't afford their medication even with insurance, Eli Lilly's Lilly Cares Foundation provides Jardiance at no cost to qualifying patients. Eligibility is based on income (generally at or below 400% of the federal poverty level), lack of prescription drug coverage, and U.S. residency.

The application requires documentation of income and insurance status. Processing typically takes 2-4 weeks. If approved, you receive a supply of Jardiance mailed directly — usually 90 days at a time, with the option to reapply.

Other Ways to Reduce Jardiance Costs

Check if your plan's formulary favors a competing SGLT2 inhibitor. Farxiga (dapagliflozin), Invokana (canagliflozin), and other drugs in the same class may be on a lower tier on your specific plan, which could mean a lower copay for a therapeutically similar medication. Ask your doctor if switching is medically appropriate.

If you're on Medicare, compare Part D plans during Open Enrollment specifically for Jardiance coverage. Some plans place it on Tier 3 (preferred brand) while others put it on Tier 4 (non-preferred) — the difference can be $50+ per month in copays. The Inflation Reduction Act's Medicare drug price negotiation program has been negotiating prices on select drugs.
