Not all pharmacies price the same. Here's how the major chains, warehouse clubs, and online options stack up for uninsured and cash-pay patients.

If you're paying cash for prescriptions — whether you're uninsured, underinsured, or just paying less than your insurance copay — where you fill matters enormously. The same generic drug can vary by 500% or more between pharmacies in the same town. Here's how the major options stack up.

Walmart and Sam's Club

Walmart's generic drug discount program covers roughly 300 medications at flat-rate pricing — typically $4 for 30 days or $10 for 90 days. No membership, no coupon, no insurance needed. For the drugs on their list, it's hard to beat. Sam's Club pharmacies honor the same pricing. The limitation: only generics at commonly prescribed doses, and the list can change without notice.

Costco

Costco's pharmacy prices on generics are consistently among the lowest in the country — often lower than Walmart's flat rate on drugs not covered by Walmart's program. And by law in most states, you don't need a Costco membership to use the pharmacy. The downside is limited locations and shorter hours compared to chain pharmacies.

Cost Plus Drugs

Mark Cuban's mail-order pharmacy sells generics at the actual manufacturer cost plus a flat 15% markup and a small shipping fee. For maintenance medications where you can plan ahead and wait for delivery, Cost Plus is often the absolute cheapest option — especially for less common generics where other pharmacies charge high markups. They carry over 2,000 medications. The limitation: mail-order only, no same-day fills, no controlled substances, no brand-name drugs.

Amazon Pharmacy / RxPass

Amazon Pharmacy offers competitive pricing for Prime members, and its RxPass subscription covers 50+ common generics for a flat monthly fee. If you take multiple medications on the RxPass list, the per-drug cost is very low. The limitation: requires Prime membership, limited drug list for the flat-fee program, not available in all states.

Chain Pharmacies (CVS, Walgreens, Rite Aid)

Chain pharmacies generally have the highest cash prices on generics. However, with a free discount card from GoodRx or SingleCare, prices drop significantly — often matching or approaching warehouse club pricing. Chains have the advantage of convenience: more locations, longer hours, and drive-through windows. If you're going to use a chain, always use a discount card.

Grocery Store Pharmacies
